Welcome to the 2012-13 Child Care program. Below are a list of enrollment forms that will assist you in finalizing your child's enrollment in our program. These forms are for parents who have already registered for the program and who have been given a start date. If you are interested in beginning the enrollment process, please contact our Child Care office at (805) 445-8695.

 

Some forms should be printed out and retained for your records. All other forms are required to be signed and submitted to the Child Care Center by the first day of attendance. Do NOT send any completed forms to the District Office. They all need to taken to your child care location on or before your first day of attendance.
